Work-Related Accident Reporting Procedure

WORK - RELATED ACCIDENT INSTRUCTIONS
If you are involved in a work-related accident or injury, please follow the instructions outlined below:
1. If the injured employee requires medical attention go immediately to the OCCNET (647-6228), 1825 Airport Exchange Boulevard Erlanger, KY between the hours of 8:00 a.m. and 5:00 p.m. After work hours go to the St. Luke Hospital emergency room. Please make sure that they know you are a Boone County employee so proper procedures will be followed. DO NOT show them your health insurance card.
If the employee involved in the accident does not feel medical attention is required, they must sign a Medical Waiver Form immediately, which must be submitted to the Human Resources office within 24 hours.
2. Robin Curry should be notified immediately when a work related injury requires medical attention so it may be investigated as to cause; phone - 334-2245, cell - 380-9029. If he cannot be reached, call Don Wood, 334-2123. (during office hours)
After 4:30 p.m., the supervisor in charge will determine if notification can wait until the next business day or if Robin Curry needs to be paged immediately. (380-9029)
3. The SUPERVISOR in charge, not the employee, must complete an Accident Investigation Report Form which must be submitted to the Human Resources Department within 24 hours (excluding weekends and holidays), so that an accident investigation can be conducted.
